Medical Information at Springfield Central SHS

Ensuring the health and safety of our students is a top priority at Springfield Central State High School. If your student has a medical condition, please review the following guidelines to ensure the appropriate support is in place.

Medical Action P​​​lans

If your student has a Medical Action Plan, a copy must be provided before they commence at the school. For newly diagnosed conditions, the plan should be submitted as soon as reasonably possible. Action Plans can be:

Administration of M​​edication at School

If your student requires medication to be administered during school hours, please note:

  • Medication must be prescribed by a qualified health practitioner (e.g. doctor, dentist).

  • It must be in the original container with an attached pharmacy label clearly stating the student’s name and dosage.

  • Parents/carers must complete a Consent to Administer Medication Form.

  • Option A authorises the school to administer medication.

  • Option B allows the student to self-administer medication while at school.

Even if your student is self-administering, the school requires the appropriate documentation, such as:

  • Asthma Action Plan

  • ASCIA Anaphylaxis Action Plan

  • Emergency Health Plan for Diabetes

📌 Important: If your student requires more than one medication, a separate form must be completed for each medication.

In case of an emergency, staff will provide First Aid and follow the student’s action plan. If you have any concerns, please contact the school at (07) 3470 6222 or email admin@springfieldcentralshs.eq.edu.au.

Medi​​​cal Requirements for Specific Conditions

Students with Diabetes

  • Up-to-date School Plans must be in place to ensure school staff follow recommendations from the student’s health team.

  • Registered Nurses (not based at the school) are available to support students and work with families, school staff, and health professionals.

  • Parents should provide:

    • Emergency medication

    • A current Emergency Health Plan signed by a doctor

    • A completed Consent to Administer Medication Form (Option A or B)

Students at Risk of Anaphylaxis

  • If your student is at risk of anaphylaxis, parents must provide:

    • Emergency medication (e.g. EpiPen)

    • A current ASCIA Anaphylaxis Action Plan, completed by a doctor

    • A Consent to Administer Medication Form (Option A or B)

Students with Asthma

  • If your student has asthma, parents must provide:

    • Emergency medication (e.g. Ventolin)

    • A current Asthma Action Plan, completed by a doctor

    • A Consent to Administer Medication Form (Option A or B)
